You don't need to figure it out alone. Our care coordinators assess the patient's clinical needs, insurance, location, and preferences — then recommend the appropriate services. We can coordinate one service or multiple at once.

Are all of these services available throughout California?

Availability varies by region, but our network spans 16 counties and continues to grow. We're transparent about what's available in your area and will work to find alternatives when a specific service isn't nearby.

Is there a cost to use Patient Path Hub's coordination services?

No. Our care coordination is free for patients and families. There is no obligation and no commitment. The care itself is billed through the patient's insurance or self-pay directly with the provider.

How quickly can services be arranged?

Timing depends on the patient's needs, location, insurance, and provider availability. For urgent referrals we prioritize same-day or next-day coordination. Our team responds to all inquiries within one business day.

Can I coordinate multiple services at once?

Yes. Many patients need several services — for example, home health plus medical equipment plus transportation. Our coordinators manage the whole bundle so families don't have to juggle multiple agencies.
