This type of care is recommended for patients who meet the following criteria. Our coordinators can help determine eligibility based on your specific situation.
- Patient has a terminal diagnosis with a prognosis of 6 months or less
- Symptoms cannot be adequately managed at home
- Family caregiver is unable to provide the level of care needed
- Patient desires a dedicated facility rather than home-based hospice
